Physical changes and repairs so your home fits your health needs.

Home accessibility modifications

Ramps, grab bars, handrails, widened doorways, and bathroom or kitchen changes for members with a physical disability that limits independence at home.

Mold and pest removal

Fixing leaks, removing damp materials, and sealing the gaps pests use to get in, for members whose health is affected by conditions in the home.

Home heating and cooling support

Air conditioners, heaters, humidifiers, dehumidifiers, or air filters for members managing a chronic condition or recovering from a heat- or cold-related illness.

Medical refrigeration

A small refrigeration unit for medication that must stay cold, breast milk storage for pregnant or postpartum members, or nutrition formula.

Asthma home safety repairs

A broad set of home repairs and cleaning supplies for members with asthma, from air duct and mold work to HEPA vacuums and allergy-safe bedding.

Asthma education

One-on-one health education for you and your family, delivered in your home in person or by video, to help manage asthma day to day.

Navigator services are available to Staten Island Medicaid members and families, including those in select Brooklyn and Queens zip codes (11209, 11219, 11220, 11228, 11355, 11368, 11373, and 11377).
