How to File a Grievance

If you believe Ideal Health Partners has failed to provide these services or has discriminated in another way on the basis of race, color, national origin, sex, age, or disability, you can file a grievance with our compliance contact:

You can file a grievance in person, by mail, by phone, or by email. If you need help filing a grievance, our compliance contact is available to assist you. Our process is:

  1. Submit your grievance, in writing or orally, within 60 calendar days of the incident.
  2. We will acknowledge receipt within 5 business days.
  3. We will investigate and provide a written decision within 30 calendar days.
  4. If you disagree with the decision, you may request further review by outside legal counsel retained by Ideal Health Partners within 15 calendar days of the decision.

Ideal Health Partners will not retaliate against anyone who files a grievance or cooperates with a grievance investigation.

How to File a Complaint with the Federal Government

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights (OCR), electronically or by mail or phone. Filing a complaint with OCR does not require you to first file a grievance with us, and you may do both.
