Our Medicare-certified team of nurses, therapists, and home health aides delivers skilled clinical home health care services at the same quality you’d expect in a hospital, in the comfort of your own surroundings. Traditional Medicare pays 100% of our services with no deductibles, copays, or cost-sharing for qualified patients. If your doctor has ordered home health care, we can likely help.
To qualify for home health services paid 100% by Medicare, you must have a medical need for the skilled services of a nurse or therapist. You must meet Medicare’s definition of homebound. This includes the requirement that leaving the home requires the assistance of another person or the use of an assistive device. You can be temporarily homebound due to illness or injury. Your Agency must also receive orders from your doctor to provide home health services. Upon request from you or referral from your doctor, we will coordinate with your doctor directly. We can provide home health services in assisted living facilities as well as in private residences.
After a hospital stay, surgery, or medical event, your physician or discharge planner sends us a referral.

Your physician needs to order home health services and certify that you meet the eligibility criteria. This usually happens at hospital discharge or during an office visit after a significant health event. Once an order is in place, contact us and we’ll verify your benefits, confirm eligibility, and schedule your first visit — often within 24 to 48 hours.
We verify benefits for all insurance plans before services begin, including Medicare Advantage. Coverage under MA plans varies by carrier and plan, so we’ll confirm your specific benefits before your first visit to ensure there are no surprises. Traditional Medicare (Parts A and B) covers our services at 100% for qualifying patients.
Yes. Medicare-covered home health services require a physician order and a certified plan of care. Your doctor must document that you are homebound, that you require skilled services (nursing or therapy), and that home health is medically appropriate. If you were recently discharged from a hospital or rehab facility, this referral process typically happens automatically — we’ll coordinate directly with the discharging team.
