Our Commitment to Your Privacy
Ideal Home Health Care (“Ideal Home Health,” “we,” “us,” or “our”) is a Medicare-certified skilled home health agency and a HIPAA covered entity. We are committed to protecting the privacy of your protected health information (PHI). This Notice describes how we may use and disclose your PHI, and your rights regarding that information.
Information We Collect
- Identifying information (name, address, phone, date of birth, Medicare/insurance ID)
- Clinical information, including diagnoses, physician orders, plans of care, and visit documentation
- Billing and insurance information
- Information submitted through online forms on this website
Third-Party Service Providers We Use
We work with the following organizations to deliver and administer services. These providers may receive limited information as necessary to perform their function on our behalf:
- Alora — electronic health record (EHR) system used for clinical documentation and Medicare billing
- Hipaatizer — secure intake form platform used on this website for new patient inquiries
- Medicare and applicable Medicare Advantage plans — claims processing and coverage verification
How We Use and Share Your Information
We use and disclose your PHI for treatment, payment, and health care operations, including:
- Coordinating your care among your nurses, therapists, and aides
- Billing Medicare and other applicable payers
- Communicating with your physician regarding your plan of care
We may share relevant treatment information with your referring physician, other treating providers, or care team members as necessary for your treatment and continuity of care, consistent with applicable law. We do not sell your personal information. We disclose PHI beyond the purposes above only with your authorization or as required by law.

- The right to request access to and a copy of your medical records
- The right to request an amendment to your records
- The right to request an accounting of certain disclosures
- The right to request restrictions on certain uses or disclosures
- The right to request confidential communications
- The right to a paper copy of this Notice upon request
- The right to file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated
Data Security
We maintain administrative, technical, and physical safeguards designed to protect your information from unauthorized access, use, or disclosure.
Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as PHI we receive in the future. The current Notice will be available on this website and at our office.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at ocrportal.hhs.gov/ocr/cp/complaint_frontpage.jsf. You will not be retaliated against for filing a complaint.
