Our Commitment to Your Privacy
Ideal Home Help (Home Aide Today Co. d/b/a Ideal Home Help, “Ideal Home Help,” “we,” “us,” or “our”) is a HIPAA covered entity. We are committed to protecting the privacy of the personal and health information of the beneficiaries and caregivers we serve under Michigan’s Medicaid Home Help Program. This Notice describes how we may use and disclose your information, and your rights regarding that information.
Information We Collect
- Identifying information you provide (name, address, phone, date of birth, Medicaid ID)
- Health and functional information related to your care needs and eligibility
- Caregiver information, including background check results and employment records, for individuals employed by Ideal Home Help
- Information submitted through online forms on this website
Third-Party Service Providers We Use
We work with the following organizations to deliver and administer services. These providers may receive limited information as necessary to perform their function on our behalf:
- Michigan Department of Health and Human Services (MDHHS) and its CHAMPS system — program administration, eligibility, and billing
- HHAeXchange — Electronic Visit Verification (EVV), as required by MDHHS for all Home Help providers; also used by Ideal Home Help for caregiver payroll processing and case management
- ICHAT (Internet Criminal History Access Tool, Michigan State Police) — caregiver background check screening
- Hipaatizer — secure intake form platform used on this website for new consumer and caregiver inquiries
How We Use and Share Your Information
We use and disclose your information for treatment, payment, and health care operations, including:
- Coordinating your care and caregiver assignment
- Billing MDHHS for services rendered
- Verifying caregiver eligibility, training completion, and background check status
- Complying with MDHHS reporting and audit requirements
We do not sell your personal information. We disclose information beyond the purposes above only with your authorization or as required by law.
Your Rights
- The right to request access to and a copy of your records
- The right to request an amendment to your records
- The right to request an accounting of certain disclosures
- The right to request restrictions on certain uses or disclosures
- The right to request confidential communications
- The right to a paper copy of this Notice upon request
- The right to file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated
Data Security
We maintain administrative, technical, and physical safeguards designed to protect your information from unauthorized access, use, or disclosure.
Changes to This Notice
We reserve the right to change this Notice and to make the revised Notice effective for information we already have as well as information we receive in the future. The current Notice will be available on this website and at our office.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at ocrportal.hhs.gov/ocr/cp/complaint_frontpage.jsf. You will not be retaliated against for filing a complaint.
